Please input both 1. and 2. as noted below.

• “Code” and “Number” of insured person
  For “Number” with 2-digit suffix number assigned, no need to input 2-digit suffix number.
  e.g.: If number is 1234567 (suffix) 00, then input 1234567 only.

  or
  Citi SOEID（Citi employee only）

• Authentication Key
  Birth date of insured person (YYYYMMDD)

How to know “Code” and “Number”

Such information can be confirmed in one of the following ways.

■Citi Employee and for those who login by SOEID.

Code and Number is indicated at the login box after you logged in by using your SOEID.

■For all CHIS insured members

• Myna Health Insurance Card (from the health insurance section of Myna Portal)
  
• Certificate of CHIS membership information「資格情報のお知らせ」
  (Mailed to your home by CHIS)
  
• Health insurance card
  (Only for insured members who joined CHIS before December 1, 2024)
